Notice of Intent to Appear by Telephone | This Notice of Intent to Appear by Telephone must be filed with the court a minimum of 3 days (perhaps sooner - check local court rules) before the appearance and served on all parties by the close of the next business day. See California Code of Civil Procedure Section 367.5 for types of hearings appropriate for telephonic appearances. Judges may refuse to allow a telephonic appearance. Arrangements must be made directly with the court or through a court appointed vendor such as Court Call. Vendors charge a reasonable fixed charge. | |

If you are requesting a monetary judgment, you must specify the allowable costs you are claiming, and if you are requesting any judgment, you must declare the non-military status of the party against whom you are seeking judgment. Request for Dismissal | File this document when you wish to dismiss a complaint, cross-complaint, specified parties, specified causes of action - with or without prejudice as appropriate. If court costs and fees were waived for a party being dismissed, the Declaration Concerning Waived Court Fees on page 2 must be completed. Any court lien must be paid before the dismissal will be entered. Obtaining legal advice is advisable before dismissing a case. | |
